Family Medical Leave Act

The FMLA process is initiated by the employee and approved by the supervisor.

WH-381, which delineates the process for submitting the FMLA request
WH-380F – Medical Certification for Family Member 
OPM 71 – To be provided by the employee along with WH380F
WH-382 – Designation Notice – to issue after approval/disapproval or request for additional information
